Pre Survey Information

Pre-Survey Information for Clients.

What you do!
It is simply a matter of filling in the questionnaire with all your health and dietary information, and within a few days you receive your report. What could be easier? You will need to provide us information on your dietary habits. This can be either a typical day, to give us an idea of your diet, or every day for up to a week to give us more detailed information. The choice is yours. We advise you use our Food Diary Template to record information for this.

What you get!
Your report will consist of 12 pages and cover the following areas:

 Page 1 – Front page detailing your main areas of attention and report contents.
 Page 2 – Your health analysis (overview of concerns and specific nutritional advice).
 Page 3 – Your metabolic type (overview of profile and specific nutritional advice).
 Page 4 – Dietary analysis - Macro nutrient status.
 Page 5 – Dietary analysis - Micro nutrient status.
 Page 6 – Avoid & increase sheet with colour coded food options.
 Page 7 – Menu plan - explanation.
 Page 8 – Menu plan - training days (including quick ‘on the move’ options).
 Page 9 – Menu plan - non-training days (including quick ‘on the move’ options).
 Page10 – Additional information (recommended tests, supplements & further reading).
 Page 11 – Appendix 1 - The Glycaemic Index, what it means to you.
 Page 12 – Appendix 2 - Macro & micro nutrients and their role in your health.

Price
The price for this service is £47.50. This can either be paid to your trainer or health club, or directly to PT-Nutrition. You are free to complete your questionnaire at any time but we must receive confirmation of payment before a program is sent out.

Important

The questionaire form may take some time to load, please be patient. Once you have completed the questionaire, we will e-mail you your full report in just a few days.
